Patient further applied lunch servings to body. Patient sedated, restrained and cleaned. Referral to Social Worker completed. Social Worker Jimmy Hoffa's notes: Patient oriented to neither time or place, dialated pupils, mumbling inaudible chants, clearly medication seeking, experiencing both auditory and visual hallucinations, delusional.
